
Becoming a Participant
You may be referred to the program by a friend, relative, physician, therapist or you may contact Lifestyles Adult Day Healthcare Center directly regarding various services available and to schedule an intake interview.
Your first step is to be evaluated during an intake interview by the staff to determine your eligibility and need for assistance. If you are dealing with conditions and diseases that endanger your quality of life and continued independence, you may qualify for program services.
To qualify you must need health care/or therapy services but not require 24-hour care.
Once the intake is complete you will be scheduled for a home visit from our Social Worker. A medical history is obtained from your personal physician or from an exam by the staff physician. Detailed assessments are conducted at the center by the clinical staff to determine exactly which services are needed and how frequently you would need to attend. After you qualify, you are counseled on your plan of care, your attendance is arranged and transportation is scheduled as necessary.
Program Staffing
A Multi-disciplinary Team of trained professionals conduct the assessments, provide care and maintain tracking and progress reports on each individual. Helps individual maintain and/or restore optimum level of health. Rehabilitates participants to the maximum extent possible.
Assists individuals achieve independent living and delay or prevent premature placement in long term care. Family benefits include respite from 24-hr care giving responsibilities, quality medical care for participants, socialization and therapeutic activities, on-going medical and psycho-social assessments, and counseling on care giving, family issues and other concerns
